All patients diagnosed with eligible chronic diseases, such as asthma, COPD, diabetes etc are recalled each year for a review. We will generally invite patients in their birth month and if you suffer from multiple conditions we will try where possible to cover them within the same review. Invitations will usually be sent out via text, but we do also send letters if text is not available.
These are important reviews aimed at supporting you by providing education, encouraging shared decision making and giving you the confidence to manage your health. Please respond when you receive your invitation to book your review.
For some diagnosis, such as diabetes, if you do not attend your review it may impact on our ability to prescribe some medications for you. We hope that you will attend, however, if you do not wish to have your review when invited please contact the surgery to let us know.
